5 Innovative Things LightBridge is Doing to Improve Hospice Care

At LightBridge Hospice & Palliative Care, we believe that by providing an individualized approach to care and creative specialty programs, we can give our patients more control of their journey, and provide them a voice in how they are cared for during their final days. It is about honoring and respecting their lives, and giving them the opportunity to actively participate in their care and assuring their wishes are honored and respected. 

To create a customized plan of care for each patient, we have several interview questions that we ask patients and their families so that our team can get a better understanding about their preferences and deliver the most personalized care possible. It is always our priority to take the time to really get to know everyone we care for, and through our discussions, we are better able to identify which one of our innovative programs will help to improve the overall level of hospice care we provide.

From our years of experience caring for hospice patients, we have developed specialized programs that allow us to provide an environment of comfort that is unmatched. These programs, which are created to uniquely address our patients’ needs, include:

Integrative Therapies

LightBridge has a very strong Integrative Therapies Program that is available to our patients. Our Integrative Therapies currently consist of:

  • Aromatherapy
  • Pet Therapy
  • Music Therapy
  • Music & Memory (a part of our dementia program)
  • Healing Touch™
  • Reiki
  • Healing Art

Each program offers a unique approach to providing patients with a sense of peace and comfort.

Ohr Ami, The Jewish Hospice Program

Our Jewish Hospice Program, Ohr Ami, focuses on respecting, understanding, and honoring Jewish customs and traditions. Our team is lead by a Rabbi who oversees our program and provides spiritual care and guidance to our Jewish patients. And our entire team is educated in the religious traditions, culture, and history of Judaism, so they are familiar with the unique needs and concerns of the Jewish patients.

We Honor Veterans Program 

LightBridge was the first hospice program in California to achieve the prestigious Partner Level IV status in the nationally acclaimed We Honor Veterans Program, which was created by a joint initiative between the Veterans Administration and the National Hospice and Palliative Care Association. To accomplish this significant achievement we designed a program to assure our team members and volunteers were thoroughly educated in the unique needs of veterans including the customization of care plans and programs designed to address their specific service experience

Our veterans have given and sacrificed so much for our country, and we believe in honoring their lives and making sure their final days are filled with dignity and respect that honors their service. 

Heart to Heart

Our Heart to Heart program was created specifically to meet the needs of individuals living with end-stage cardiac disease. It is designed to meet the special needs of cardiac patients and their loved ones, reduce the number of emergency room visits, and manage pain and other symptoms of the disease. With our Heart to Heart program, our goal is to minimize the complications of the disease and maximize the comfort of our patients. Our team works with our cardiac patients, their loved ones, and caregivers to create a customized plan to keep them as comfortable as possible in their final days. 

Dementia Program

LightBridge provides a specialized program for our patients with dementia that focuses on specific ways to approach their care and treatment. We recognize the unique needs and challenges of dementia patients, and our staff and volunteers are trained and educated to become dementia specialists. It is our goal to meet their medical and social needs while giving them peace and a sense of control over their care. Our dementia program focuses on who the patient was before the disease in order to connect with them and provide the best care possible. 

All of our innovative programs are meticulously designed and planned. We put an enormous amount of detail and effort into these programs because they are at the center of how we provide care to our patients.
